Key Responsibilities

• Perform comprehensive medication reviews (cmrs) and targeted medication reviews with patients.
• Identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain compliance with cms mtm program requirements and quality metrics.

What You'll Bring

• Education: Pharmd or bachelor of pharmacy.
• Licensure: Active u.s. pharmacist license.
• Experience: Ambulatory, hospital, community, or prior mtm experience is a plus. All licensed pharmacists, including new graduates, are encouraged to apply.
• Skills: Excellent patient counseling, independent work style, and clear documentation.
