Social Worker

Join our team at Aspen Hill Rehabilitation and Healthcare Center as a Licensed Social Worker (LSW) Full-time (40 hours) Salary range $25-$50 hr. Responsibilities Evaluate patients (existing, new and prospective) based on their needs and limitations Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager Build rapport with patients and their families Coordinate care with PT departments Develop care plans for patients Qualifications Must have long term care experience and active Social Work license in the state Must possess the ability to make independent decisions when circumstances warrant such action Must be knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post-acute care facility Must have the ability to implement and interpret the programs, goals, objectives, policies, and procedures of the department Must perform proficiently in all competency areas including but not limited to general social services responsibilities, patient rights, and safety and sanitation Displays integrity and professionalism by adhering to the Marquis Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training Benefits Tuition reimbursement Employee referral bonus Health, vision, and dental benefits 401(k) with match Employee engagement and culture committee Company sponsored life insurance Employee assistance program (EAP) resources Join our team at Aspen Hill Rehabilitation and Healthcare Center, a 127-bed Sub-Acute, and Long-Term Care facility where compassion and quality care are at the heart of everything we do.
