Job Description
Provides personalized education, training, and support to patients with diabetes or prediabetes and other chronic diseases, including hypertension and COPD focusing on self-management behaviors. Develops and delivers diabetes and other chronic disease education plans, teaches lifestyle modifications, and instructs patients on glucose monitoring, medication management, proper nutrition, and exercise.

We were one of the five clinics which founded the Texas Association of Charitable Clinics. Come join us and our Mission which to minister in a caring and Christian environment to the medically underserved of Kerr and surrounding counties. Everyone deserves quality care. Our staff is passionate about what they do. Join our patient care team and make a difference with an underserved population, the majority of whom speak Spanish!
