Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Cardinal Woods Skilled Nursing and Rehab in Madison, Ohio, USA. As an LPN, you will play a vital role in providing high-quality patient care, promoting a positive and supportive environment, and working collaboratively with our interdisciplinary team.

Responsibilities:

Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
Monitor patients' conditions and report changes to physicians and other healthcare professionals
Assist patients with activities of daily living, such as bathing, dressing, and grooming
Develop and implement individualized care plans for patients
Collaborate with other healthcare professionals to ensure comprehensive care
Maintain accurate and up-to-date patient records
Participate in quality improvement initiatives to ensure continuous quality of care

Current LPN licensure in the state of Ohio
Strong communication and interpersonal skills
