Minimum Qualifications Associate degree in Physical Therapy from an accredited educational program. Active California PTA license required to start the assignment. BLS certification from AHA or ARC required prior to start. Minimum of 1 year of experience working in a skilled nursing facility, long-term care, or post-acute rehabilitation setting preferred. Experience providing restorative and skilled physical therapy services to geriatric patients preferred. Familiarity with documentation in electronic medical record systems and Medicare/Medicaid skilled nursing documentation requirements preferred. Ability to collaborate with interdisciplinary teams and follow plan of care set by supervising PT.

Assignment Details Facility Type: Skilled Nursing Facility. Shift Name: Days. Shift Details: 5 days per week. Start timeframe: mid June start window; facility requests two clinicians (two PTAs, two PTs, or one of each).
