Elara Caring invites a dedicated Hospice Registered Nurse to join our team as a Full-Time RN Case Manager. You will assess patients and families, coordinate care, and educate staff across nursing homes, hospitals and home settings. We seek an RN with a current license, 1+ year clinical experience, and willingness to travel for patient visits. Hospice/home health experience is preferred.
