Job Summary The Physical Therapist (PT) contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. Responsibilities Assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s). Provide patient, caregiver, and family counseling. Provide patient and caregiver education. Prepare clinical notes, and progress summaries based on the attainment of goals. Provide physical therapy services to patients according to a written physician's plan of care. May train patient in the use of prosthetic device. Identify patient and family/caregiver needs for other home health services and refer as necessary. Participate in discharge planning for patient. Supervise Physical Therapy Assistants according to organization policy and state regulations. Qualifications Licensed to practice as a physical therapist within California. Demonstrates good verbal and written communication and organization skills. Possesses and maintains current CPR certification. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
