FIELD REGISTERED NURSE | Manhattan, NY

Key Responsibilities

Conduct home visits to patients
Complete clinical documentation and care plans
Coordinate care with primary care physicians
Update medication profiles
Communicate with other healthcare professionals involved in patient care
Maintain case notes and follow-ups using systems like HCHB
Participate in case conferences with supervisors

Requirements

Active Registered Nurse (RN) license
At least 6months of experience working as RN
Prior experience in home care or clinical nursing is preferred
Ability to travel to patient homes
Strong documentation, communication, and organizational skills
