Hospice On-Call Registered Nurse (RN) - PRN

Location: Spring, TX Area

At Concho Hearts Hospice , our On-Call Nurses provide comfort, support, and skilled care to patients and families when they need it most. Hospice nursing is about more than tasks; it's about being present, providing reassurance, and helping patients receive compassionate care wherever they call home.

We are seeking a compassionate On-Call RN to support our patients and families in the Spring area.

What You'll Do:

• Respond to after-hours patient and family calls with compassion and professionalism.
• Triage patient concerns and determine appropriate interventions.
• Complete urgent patient visits as needed in homes or facilities.
• Provide symptom management and end-of-life nursing support.
• Collaborate with physicians and the interdisciplinary hospice team.
• Maintain accurate and timely documentation using the EMR system.
• Support patients and families through education and guidance during after-hours care.

What We're Looking For:

• ActiveTexas RN license.
• Hospice, home health, ER, ICU, or acute care experience preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently and make sound clinical decisions.
• Excellent communication and compassionate patient care skills.
• Reliable transportation and willingness to travel locally.
• Current CPR/BLS certification.
