Key Responsibilities\n

  • Deliver individualized therapy sessions, including individual, family, and group therapy, using evidence-based treatment modalities tailored to hospice care.

  • Implement various therapeutic approaches, such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care, to address the complex emotional needs of clients.

  • Design and execute personalized treatment plans, focusing on the unique circumstances of each client, including specific goals and therapeutic interventions.

  • Work collaboratively with a multidisciplinary team, including psychiatrists, psychologists, and medical professionals, to provide integrated client-centered care.

  • Conduct thorough crisis interventions, risk assessments, and safety planning as needed to ensure the well-being of clients.

  • Maintain comprehensive, accurate documentation in accordance with ethical, legal, and organizational standards.

  • Lead group therapy sessions, psychoeducation workshops, and support groups aimed at enriching client coping skills and resilience.

Patient Population / Client Focus\n

In this role, you will primarily work with adults receiving hospice care, along with their families. Your responsibilities will involve providing emotional support and dealing with sensitive issues related to grief, loss, and adjustment to terminal illness. Adapting your therapeutic style will be essential, as you engage with clients from diverse backgrounds, each with unique experiences and perspectives on end-of-life care.

Qualifications\n

  • A Master’s degree in Social Work from an accredited institution.

  • Current active licensure as a Licensed Clinical Social Worker (LCSW) in New York.

  • A minimum of two years of experience in a mental health or outpatient setting, preferably in hospice or palliative care.

Compensation and Benefits\n
