Role Details Role: Nurse Practitioner (NP) – General Medicine Assignment Length: 13 weeks (ongoing potential) Schedule: Days, PRN (half‑day or full‑day options) Location: Kokomo, IN area (provider must be within 50 miles or have a local office) Office Requirements Provide clear photos documenting clinic signage; parking with a designated accessible space and visible wheelchair route; front/suite entrances and thresholds; elevator if not on the ground floor; waiting room furniture; and the exam room showing the door threshold, exam table with protective paper, and provider workstation. Also include an accessible restroom with grab bars and full sink/toilet view, a fire extinguisher with readable inspection/manufacture details, illuminated emergency exits, and evidence of single‑use PPE, medical supplies, and basic medical instruments (e.g., blood pressure monitor). Qualifications and Requirements Active, unrestricted Nurse Practitioner license (Indiana) AANP Certification (required) Experience in general medicine, including METs testing and respiratory & cardiovascular evaluations Must own a Windows laptop (no Mac/Apple devices) Willingness to complete DMA training & certification Must not be actively performing similar assessments elsewhere Onboarding Simple, stress‑free onboarding.
