CHC Service Coordinator Supervisor

UPMC

UPMC is seeking a CHC Service Coordinator Supervisor for Southeast, PA to lead a team supporting Community HealthChoices members. In this role, you will supervise service coordinators who assess member needs, develop person-centered care plans, and coordinate medical, behavioral health, and social services. You will monitor caseloads, quality, and compliance with state and UPMC policies, provide coaching and training, review documentation, and use data to drive improvements. The supervisor collaborates with providers, community agencies, and families to ensure timely, high-quality, home- and community-based services that promote independence and wellness.
