Direct Support Specialist
Mosaic Life Care
Mosaic Life Care is seeking a compassionate Direct Support Specialist to provide person-centered care to patients in our healthcare and social assistance setting. In this role, you will support daily living activities, observe and report patient conditions, assist with care plans, and ensure safety and dignity for each individual served. Working within a collaborative clinical team, you’ll document care, follow individualized support plans, and promote independence. Mosaic Life Care offers a culture of compassion, innovation, and professional growth while you help improve the lives of patients and the broader community.
