Key Responsibilities for a Concierge Physician:

· Deliver comprehensive, relationship-centered medical care in patients’ homes and other appropriate settings, supplemented by telehealth visits.

· Perform assessments, preventive care, chronic disease management, medication review, and clinically appropriate diagnostic evaluation.

· Coordinate with specialists, caregivers, families, and other clinicians while maintaining clear clinical judgment and patient confidentiality.

· Maintain accurate, timely clinical records and collaborate with the Shorecliff care coordination team.

· Participate in a shared, limited weekend coverage schedule and travel to assigned service areas as needed.

Qualifications:

· Current California medical license and appropriate prescribing authority.

· Strong communication skills, professional discretion, and comfort working independently in a mobile-care environment.

· Ability to travel locally for house calls; reliable transportation and valid driver’s license.

· Concierge medicine, home visits, geriatrics, or high-touch primary care experience preferred, not required

The Opportunity:

We are seeking compassionate, clinically strong professionals interested in a fully concierge model outside routine Medicare and commercial insurance billing. We welcome candidates who already practice in this model, as well as those open to exploring the professional and regulatory implications of transitioning to it.

Our focus is thoughtful, relationship-based care with time for meaningful patient interactions, primarily delivered through home visits and supported by telehealth.

Cash Pay/Medicare Participation:
This position is designed for a cash-pay concierge practice, not routine Medicare or commercial insurance claim billing. Applicants should be willing to discuss a lawful transition to this model, including Medicare opt-out when applicable and permitted, and any impact on other clinical positions or payer relationships. Medicare opt-out is subject to CMS rules; eligible clinicians should not change their status before understanding the requirements and effective dates.
