Job Title: Licensed Practical Nurse
Service Location: Lake Ariel, PA
Rate: $28-$32 hours per hour

1) Immediate Shift Opening: Looking for school accompaniment 7:30am-4:30pm, Wallenpaupack school district

2) Immediate Shift Opening : Saturday & Sunday 7am-11pm, shifts can be split/Monday, Thursday,& Friday 6pm-6am

3)Immediate Shift Opening: LPN or HHA school hours 7am-4pm, home hours 9pm-7am
Rate: $28-$32 per hour

Responsibilities:

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
