Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, while aligning with department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups.
- Assess patient progress and adjust physical therapy plans to optimize outcomes.
- Document evaluations, treatment plans, and progress notes with accurate and timely records.
- Educate patients and caregivers on home exercise programs, safety, and self-management.
- Collaborate with other health professionals to coordinate care and support patient recovery.

Requirements
- Ability to operate within an interdisciplinary care model and communicate effectively with the team.
- Strong documentation and patient-education skills.
- Availability for a day shift schedule (8-hour days) and a 13-week travel assignment.
- Willingness to work in a healthcare facility environment in the specified location.
- Eligibility to participate in a travel assignment and comply with assignment-specific requirements.
- Ability to adapt to evolving patient needs and maintain professional standards of care.
