Clinical Pharmacist – Home Infusion & Sterile Compounding

Milwaukee, WI | Contract-to-Hire Opportunity

We are currently hiring a Clinical Pharmacist for a growing home infusion pharmacy in Milwaukee, Wisconsin. This position offers the opportunity to combine clinical expertise with hands-on sterile compounding responsibilities in a patient-centered environment. The role begins as a contract assignment with the potential to transition into a permanent position after a minimum six-month engagement.

Schedule
Monday through Friday
8-hour shifts scheduled between 7:30 AM and 6:00 PM
On-call rotation includes one weekday evening per week and every fourth weekend
Position Summary

The Clinical Pharmacist will support the delivery of home infusion therapies through prescription verification, sterile IV compounding oversight, and clinical pharmacy services. The ideal candidate will have recent experience in home infusion and be well-versed in sterile preparation standards and infusion-related therapies.

Essential Duties

Review and verify infusion medication orders to ensure clinical appropriateness and patient safety
Perform and oversee sterile IV compounding activities in accordance with pharmacy regulations and quality standards
Provide clinical support for patients receiving home infusion therapies
Collaborate with pharmacists, technicians, nurses, and healthcare providers to coordinate care
Monitor compliance with sterile compounding protocols and regulatory requirements
Assist with workflow management and pharmacy operations as needed
Contribute to a culture of safety, accuracy, and exceptional patient care

Required Qualifications

Active Wisconsin Pharmacist license in good standing
Minimum two years of recent sterile compounding experience
Recent experience working in a home infusion pharmacy setting preferred
Demonstrated proficiency with IV admixture preparation and clean room procedures
CPR certification is a plus
Strong clinical assessment, communication, and problem-solving skills
