Job Title: Occupational Therapist – Home Health

Job Type: Contract (13 Weeks)

Work Schedule: Day Shift – 5x8 Hours (8:00 AM – 4:30 PM)

Location: Medford, OR

Job Summary:

We are seeking an experienced Occupational Therapist (OT) for a home health setting. The ideal candidate will provide one-on-one patient care, helping individuals improve daily living skills and overall independence in their home environments.

Key Responsibilities:

Evaluate patients and develop individualized treatment plans
Provide in-home occupational therapy services
Monitor patient progress and adjust treatment plans as needed
Complete accurate and timely documentation using EMR systems
Educate patients and caregivers on therapy techniques and home safety
Manage a daily caseload while maintaining productivity standards
Travel between patient homes within the assigned service area

Patient Care Expectations:

1:1 patient ratio
Up to 6 patient visits per day
Productivity expectation: ~30 points per week

Qualifications:

Active Occupational Therapist (OT) license (or eligibility)
Minimum 1 year of skilled home health experience required
Experience with OASIS documentation required
Strong communication and time management skills
Ability to work independently in a home health environment

Certifications:

BLS certification required
