Field-Based Geriatric Care Manager Senior Living

Bluestone Physician Services is seeking a Geriatric Care Manager to support complex, chronically ill patients in senior living communities across Minnesota. The role links patients, families, and primary care to ensure a unified care plan, reducing hospitalizations and optimizing outcomes. The position is primarily field-based, with 90% travel to Forest Lake, Hugo, and surrounding areas, and standard daytime hours Monday–Thursday 8am–5pm, Friday 8am–3pm.
