We are seeking a dedicated and compassionate LPN Nursing Team Lead to join our team at Oxford Health & Rehabilitation Center. About the Role:

As an LPN Nursing Team Lead, you will be responsible for overseeing the daily operations of our nursing department, leading by example, and promoting a positive work environment. Your primary focus will be on delivering high-quality patient care, ensuring that our patients receive the highest level of attention and support. You will work closely with our interdisciplinary team to develop and implement individualized care plans, addressing the unique needs of each patient.

Responsibilities:

• Lead a team of licensed practical nurses (LPNs) and certified nursing assistants (CNAs) to ensure seamless delivery of patient care

• Develop and implement care plans in collaboration with the interdisciplinary team

• Monitor patient progress and identify areas for improvement, making necessary adjustments to care plans

• Provide guidance and support to LPNs and CNAs to promote best practices in patient care

• Participate in quality improvement initiatives to optimize patient outcomes

• Collaborate with other departments to ensure smooth transitions of care

• Maintain accurate and timely records of patient care and progress

• Provide education and training to staff members to enhance their knowledge and skills

Requirements:

• Current LPN licensure in the State of North Carolina

• Minimum 2+ years of experience in a long-term care setting

• Strong leadership and management skills

• Excellent communication and interpersonal skills

• Ability to work effectively in a fast-paced environment

• Strong attention to detail and organizational skills

• Ability to lift, transfer, and reposition patients as needed
