Job Title: Holston Valley Medical Center-AP-Physician-0004 Location 130 W Ravine Rd, Kingsport, Tennessee 37660, United States State Tennessee Schedule Nights, Evenings. Shift Details: MD – Day 1: 7 am‑5 pm MD – Day 2: 7 am‑5 pm MD – Day 3: 7 am‑7 pm MD – Day 4: 7 am‑7 pm MD – Night 1: 7 pm‑7 am NPPA – Day 1: 10 am‑8 pm NPPA – Night 1: 8 pm‑6 am Average patient volume: 93 encounters/day total. Qualifications NP Requirements Graduate of an accredited Nurse Practitioner program. Preferred: 2 years of recent experience in the specialty; new graduates may be accepted depending on site needs. Active unrestricted TN RN License, TN NP License, TN DEA License with TN address & Prescriptive Authority. Active certification through AANP/ANCC. Merit Badge: ACLS, PALS. Immunizations: Documentation of MMR/Varicella (2) vaccines or titers indicating immunity; Two‑Step TB Test within last 12 months; if positive, Chest X‑ray within last 3 months; TB QuantiFERON Gold test result indicating immunity within last 3 months. Case logs for past 2 years. CME/CEU: Preeclampsia‑Eclampsia CME required during initial application and every two years for reappointments. Specific facility procedures: N/A. Malpractice Coverage: $1 M per loss event / $3 M annual aggregate. PA Requirements Graduate of an accredited Physician Assistant program. Preferred: 2 years of recent experience in the specialty; new graduates may be accepted depending on site needs. Mentorship or Proctorship may be required. Active unrestricted TN Medical license & DEA with TN address; schedules 2, 2N, 3, 3N, 4 & 5. Active certification through NCCPA. Merit Badge: ACLS, PALS.
