Role Physical Therapist (PT) specializing in home health services, responsible for evaluating, developing treatment plans, and delivering evidence-based therapy to improve patient mobility within a specified coverage area. Responsibilities Conduct comprehensive home visits, including Start of Care, evaluations, routine revisits, and discharges; Monitor and document patient progress; Collaborate with physicians and interdisciplinary team members; Educate patients and caregivers; Ensure compliance with documentation standards and Medicare guidelines. Qualifications & Skills Valid Indiana PT license; Preferred home health experience; Compassionate, empathetic, flexible, with strong problem-solving and interpersonal skills; Familiarity with HCHB/PointCare systems; Professional attire acceptable.
