Behavioral Health Care Manager (Collaborative Care Model - CoCM)

Helena, Arkansas

Full-Time | Exempt

East Arkansas Family Health Center is seeking a compassionate and organized Behavioral Health Care Manager to join our integrated Behavioral Health team. This position plays a key role in our Collaborative Care Model (CoCM) by coordinating behavioral and physical healthcare services for patients experiencing mental health and substance use disorders.

As a core member of the interdisciplinary healthcare team, you will collaborate with primary care providers, psychiatric consultants, behavioral health professionals, and community partners to ensure patients receive comprehensive, evidence-based, patient-centered care.

Essential Duties and Responsibilities

• Coordinate behavioral health and primary care services for an assigned patient caseload.
• Screen and assess patients for common mental health and substance use disorders.
• Educate patients and families regarding behavioral health conditions and available treatment options.
• Monitor patient progress, treatment response, and medication adherence through in-person and telephone follow-up.
• Provide brief evidence-based behavioral interventions, including Motivational Interviewing, Behavioral Activation, and Problem-Solving Therapy.
• Collaborate with primary care providers and psychiatric consultants to develop and implement individualized treatment plans.
• Participate in weekly Collaborative Care case review meetings with the psychiatric consultant.
• Maintain patient registries and track clinical outcomes to ensure timely follow-up and quality care.
• Coordinate referrals to specialty behavioral health services, substance use treatment programs, community resources, and social service agencies.
• Assist eligible patients with enrollment in East Arkansas Family Health Center's Patient Assistance Program.
• Connect patients with community resources including housing assistance, transportation, financial assistance, vocational rehabilitation, and other supportive services.
• Develop relapse prevention and self-management plans for patients successfully completing treatment.
• Maintain accurate documentation in the Electronic Health Record (EHR) and other required systems.
• Participate in community outreach activities, quality improvement initiatives, and program development.
• Prepare monthly reports and other program documentation as required.
• Maintain compliance with HIPAA, OSHA, and all applicable federal and state regulations.
• Perform other duties as assigned. 

QualificationsRequired

• Current Arkansas licensure in one of the following:
  • Licensed Master Social Worker (LMSW)
  • Licensed Professional Counselor (LPC)
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Social Worker (LSW)
  • Registered Nurse (RN) (BSN preferred)
  • Nurse Practitioner (NP)
  • Licensed Psychologist
• Experience in behavioral health, care management, case management, counseling, nursing, or a related healthcare field.
• Knowledge of behavioral health assessment, treatment planning, and care coordination.
• Experience with evidence-based behavioral interventions such as Motivational Interviewing, Behavioral Activation, or Problem-Solving Therapy.
• Strong communication, organizational, and interpersonal skills.
• Ability to collaborate effectively within an interdisciplinary healthcare team.

Preferred

• Experience working in a Federally Qualified Health Center (FQHC) or community health center.
• Experience working within a Collaborative Care Model (CoCM).
• Experience with Electronic Health Record (EHR) systems.
• Experience coordinating behavioral health and primary care services.
• Bilingual skills are a plus.
