You will collaborate with physicians, families, and interdisciplinary team members to deliver high-quality, patient-centered care.Key ResponsibilitiesPerform patient evaluations within 24 hours (or 1 business day) of referralDevelop and implement individualized treatment plansProvide therapy services to improve strength, balance, mobility, and functionMonitor patient progress and adjust treatment plans as neededMaintain accurate and compliant clinical documentationParticipate in care planning, rehab conferences, and discharge planningEducate residents, families, and staff on therapy programs and goalsRecommend and assist with durable medical equipment (DME) needsConduct in-service training and staff educationSupervise PTAs, aides, and studentsQualificationsDegree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)Active, unencumbered Physical Therapist license in South Carolina (required)Must maintain all continuing education and remain in good standing with the State BoardExperience in a skilled nursing or rehab setting preferredFamiliarity with PointClickCare (PCC) and Casamba preferredStrong clinical, communication, and leadership skillsWhy Join Cherrydale Post Acute?At Cherrydale, you’re not just taking a job—you’re stepping into a career path with growth potential.
