Registered Nurse - Case Management- Case Manager

Case ManagerThe Case Manager job in Palm Springs, CA lets you coordinate patient care in a 385-bed acute-care hospital with a stroke receiving center and level II trauma designation. You will oversee clinical plans of care, integrate care coordination, utilization management, and discharge planning, and facilitate multidisciplinary patient care conferences at the facility. Required qualifications include a current California RN license, excellent organizational and communication skills, critical thinking and problem-solving abilities, and proficiency with electronic medical record (EMR) systems such as eCCM and InterQual. Recommended skills include experience with medical necessity screening, discharge planning, and collaboration with physicians, ancillary staff, and payors. A Bachelor of Science in Nursing (BSN) is preferred.
