Our Home Health RN is responsible for providing individualized goal-oriented nursing care using the nursing process in accordance with a physician s order. In addition, this position serves as Case Manager in cases in which this position is the lead discipline.

• COVID VACCINATION REQUIRED - no exemptions.
• Oasis experience is a must.
• Home health experience is required.
• Experience as a home care traveler with proficiency in OASIS is preferred, as this role includes case management responsibilities.
• Homecare Home Base experience is preferred.
