Registered Dietitian

Registered Dietitian (Part-Time) Facility: Care Connection Rehabilitation Center Location: Rahway, NJ Schedule: Part-Time | Up to 15 Hours per Week Pay Rate: $45–$50 per hour Join Our Team as a Registered Dietitian! Care Connection Rehabilitation Center is seeking a compassionate, experienced, and resident-focused Registered Dietitian to join our interdisciplinary team. In this role, you will oversee the nutritional care of our residents, develop individualized nutrition plans, ensure regulatory compliance, and help promote the highest quality of life for those we serve. What You'll Do Conduct comprehensive nutritional assessments for residents upon admission and as needed. Develop, implement, and monitor individualized nutrition care plans. Review physician orders and ensure appropriate therapeutic diets are followed. Participate in interdisciplinary care plan meetings and resident conferences. Collaborate with nursing, rehabilitation, social services, activities, and dietary departments. Complete MDS nutrition assessments and documentation. Maintain accurate clinical documentation, progress notes, and dietary records. Provide nutrition education and counseling to residents, families, and staff. Ensure appropriate meal substitutions meet nutritional requirements. Monitor food service operations for sanitation, safety, and quality compliance. Assist with state and federal surveys and implement corrective action plans as needed. Review food service costs and assist with budgeting and cost‑control initiatives. Maintain compliance with HIPAA regulations and resident rights. Participate in Quality Assurance (QA) and Performance Improvement initiatives. Qualifications Current, active, unrestricted Registered Dietitian (RD) credential in New Jersey. Bachelor's Degree in Dietetics, Nutrition, or a related field from an accredited program. Minimum of two (2) years of experience in a Skilled Nursing Facility (SNF) or long‑term care setting preferred. Previous nursing home or SNF experience required. Strong knowledge of clinical nutrition, therapeutic diets, and long‑term care regulations. Experience with SigmaCare and GeriMenu preferred. Knowledge of food service budgeting and cost management. Excellent communication, organizational, leadership, and interpersonal skills. Strong understanding of federal and New Jersey long‑term care dietary regulations.
