Responsible for promoting and restoring patient's health by completing the nursing process; collaborating with physicians and team members; and providing physical and psychological support to patients and their families. Exhibits the core values of the organization, compassion, integrity and unity. He/she practices within the boundaries of the Kansas Nurse Practice Act, specialty standards, and organizational/departmental policies and procedures.Responsibilities and Essential Job FunctionsConsistently meets work schedule.Demonstrates critical thinking utilizing all aspects of the nursing process.Maintains a safe environment for patients.Coordinates health care services with hospital, vendors, physician's offices and other institutions.Answers patient/significant others phone calls, evaluate patient's needs, and triage patient care.Provides patient/family education using resources available.Acts as liaison between patient, significant others and health team members.Identifies area for professional growth.Participates fully as a member of the Patient Centered Medical Home Team.Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.These statements are intended to describe the essential functions of the job and are not intended to be an exhaustive list of all responsibilities.
