Registered Nurse, Case Manager (Day Shift)ARMStaffing is looking for a Registered Nurse, Case Manager in Portland, Maine.Location: Portland, MEAssignment Duration: This is a 13-week contract position.Schedule/Shift: Monday through Friday, with every third weekend and holiday required. The shift is from 8:00 AM to 4:30 PM.Job Description: The Registered Nurse Case Manager will be responsible for discharge planning and care coordination for patients in a high-acuity and complex care setting. The role involves working closely with patients, families, and healthcare teams to facilitate optimal patient outcomes.Responsibilities:Conduct assessments and develop discharge plans tailored to individual patient needs.Coordinate care services to ensure smooth transitions from the hospital to home or other settings.Collaborate with healthcare teams to optimize patient discharge processes.Address and resolve potential barriers to discharge and follow-up care.Utilize electronic medical record (EMR) systems including EPIC and Wellsky for documentation.Required Experience: Minimum of 5 years of hospital case management experience and 5 years of acute adult care nursing experience is required.Certifications/Licensure: Active Registered Nurse (RN) license is required.About ARMStaffing: Founded in 2011 and a member of the ARGroup, LLC, ARMStaffing connects dedicated nurses and allied health professionals with the facilities that need them most.
