LVN / LPN - LTC/SNF

$1645.60/week –

Job Summary:
Provide compassionate nursing care to patients in a secured Alzheimer’s and Dementia facility during evening shifts. Ensure patient safety, administer medications, and monitor residents’ well-being in a secure environment.

Responsibilities:

• Administer prescribed medications and treatments accurately and safely.

• Monitor and record residents health status, behavior, and responses to treatments.

• Collaborate with healthcare team to develop and implement individualized care plans.

• Ensure safety protocols are followed, especially within secure units.

• Respond promptly to residents needs and emergencies.

• Maintain accurate documentation of patient care and shift reports.

Qualifications:

• Current LVN or LPN license issued by the state.

• Demonstrated experience in LTC or SNF settings preferred.

• Ability to work evening shifts (2:00 pm – 10:30 pm).

• Must have current COVID-19 vaccination or valid exemption documentation.

• Excellent communication and observation skills.

• Able to follow detailed care protocols and safety procedures.
