Registered Nurse (Patient Safety Manager)

The Registered Nurse (Patient Safety Manager) coordinates and implements patient safety policies/activities developed by the National Center for Patient Safety (NCPS). The Patient Safety Manager is the facility's key person to help build, implement patient safety systems, and promote the culture of safety within Veterans Health Administration (VHA) facilities for Veteran patients in all healthcare settings. Other related duties and responsibilities include, but are not limited to the following:

• Provides leadership for safety assessments, coordinates the activities of the patient safety program, educates other employees on the system-based causes for medical error, consults with management and staff, and communicates literature-based ideas regarding effective patient safety strategies to others within the organization.
• Assesses, plans, implements, and evaluates delivery of care based on age specific components.
• Directs others within the facility towards process improvements that will support the reduction of medical/health care errors and other factors that contribute to unintended adverse patient outcomes.
• Performs joint oversight of the facility-wide Patient Safety Program.
• Maintains the data components and records associated with patient safety reports, which includes the tracking and trending of data and reviews, aggregating into reports, ensuring compliance with established timeframes and conducting Root Cause Analysis (RCA) as appropriate.
• Manages and utilizes a variety of automated/software systems and supporting processes for Quality & Patient Safety (including but not limited to): Joint Patient Safety Reporting (JPSR), Web Spot, Computerized Patient Record System (CPRS), Veterans Health Information Systems and Technology Architecture (VISTA), VHA Support Service Center (VSSC), Auditing, etc.
• Responsible for the documented outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility, Veterans Integrated Systems Network (VISN), or Veterans Affairs Central Office (VACO).
• Works with leaders and others to sustain a culture of safety, modeling High Reliability Organization (HRO) practices, such as patient safety forums.
• Trains and coordinates patient safety activities of personnel within the facility, such as infection control professionals, risk managers, and quality managers.
• Participates in rounding, focus groups, and other activities to determine need for improvements in patient safety.
• Participates in the annual facility-wide risk assessment. Based on the risk assessment, collaboratively develops and implements an organizational performance/process improvement plan to address, mitigate, and minimize risk to proactively improve Veteran care.
