Reviews information from all sources (chart, team report, patient evaluation, etc.) prior to initiating treatment and completes a comprehensive therapy evaluation appropriate to the age and developmental level of the patient to identify skilled therapy needs. 2. Proficient knowledge for obtaining and interpreting information to formulate an individualized treatment plan. Understands the range of treatment appropriate to the diagnosis or developmental stage. Able to recognize the impact of diagnosis or injury on the lifespan trajectory. Demonstrates clinical strong clinical knowledge and professional skills through mentoring team members, advanced self-study and specialized continuing education activities. Incorporates evidence based practice into treatment. 3. Establishes relevant measurable goals and plan of care appropriate to problems identified and involves the patient/family according to their capabilities and desires. 4. Instructs and explains prescribed treatment to parents and/or patients. Educates the patient and family/caregiver about patient deficits. 5. Proficient with implementing the treatment plan using appropriate modalities, monitors and modifies the plan of care as appropriate, and documents findings of patient progress according to established polices and procedures. Seeks consultation as necessary. 6. Maintains a positive attitude while demonstrating the adaptability necessary to effectively respond to changing departmental needs. 7. Completes work efficiently by meeting productivity standards and administrative expectations (quality initiatives, annual eval, time off). Enters treatment charges in a timely manner and without discrepancy. 8. Demonstrates initiative and flexibility to ensure that all patient care for the occupational therapy section is completed daily.
