ATN Healthcare - - Responsibilities: Conduct telephonic pre-visit check-ins on patients assigned to your practice or practices.; Spend 7-8 hours a day on the phone with patients to solicit and document pre-visit information.; Confirm reason for visit, review medication list, social determinants of health, gaps in care, and eligibility for assistance or referrals.; Coordinate with providers and office staff to expedite the on-site rooming process.; Make approximately 30-50 phone calls a day to achieve 12-16 patient interactions and 20 minutes of registration for 12-15 patients a day.
