Job Title: Case Manager Nurse Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 1 year of home health case management experience License: Valid nursing license required Certifications: BLS required Must-Have: Proficiency in OASIS Description: Home health case management experience is required. OASIS experience is mandatory and must be proficient. Locals are allowed at the same rate. Mileage compensation will follow standard IRS rates. Candidates must have at least 1 year of relevant experience. A supervisory reference is required for submission. Candidates must prepare for an interview with hiring managers. Floating may be requested based on facility needs. All candidates require specialty experience within the last year. Certifications must be through the American Heart Association. NIH and stroke education are required for all registered nurses. Prior permanent employees must be away for at least 6 months before reapplying. All requests for time off should be kept to a minimum. No additional requests for time off will be approved after an offer has been made. Candidates are required to submit proof of relevant licenses and certifications.
