Senior QIDP: Person-Centered Care & Compliance Lead

The role leads the Person-Centered Process, coordinating Discovery Phase conversations and implementing clear, measurable plans. The ideal candidate will have a Bachelor’s in Human Services, 2+ years of relevant experience, and a path to QIDP certification within 6 months, along with a valid IL driver's license and #J-18808-Ljbffr JCFS Chicago
