Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 17:00 Experience: Minimum 1 year of relevant experience License: Must have an active nursing license Certifications: Basic Life Support (BLS) required Must-Have: - Previous charge experience - Patient ratio experience - Charting system experience - Community hospital experience - Familiarity with various healthcare guidelines and coding systems Description: We are seeking a qualified Case Manager to join our team. The ideal candidate will have a minimum of 1 year of charge experience in a relevant setting. Experience with patient ratios and various charting systems is required. Candidates must have a background in community hospital settings. Familiarity with long-term acute care, rehab, and skilled nursing is considered an asset. Knowledge of admission criteria, care coordination, and discharge planning is essential. Candidates should be able to utilize InterQual Criteria and Milliman Guidelines. Understanding of CMS guidelines, CPT coding and billing, and ICD-10 coding is important. The role involves working within HIPAA guidelines and understanding DRGs. Experience with HEDIS measures and knowledge of NCQA and OSHA regulations is preferred. The Case Manager will coordinate care and support patient needs effectively. This role involves collaboration with interdisciplinary teams to optimize patient care.
