Physical Therapist (PT)

Job Summary With autonomy and minimal direct oversight, delivers exceptional, evidence-based physical therapy services to patients, caregivers, and partner facilities in full compliance with physical therapy best practices, state licensure requirements, Medicare guidelines (including general supervision of assistants in applicable outpatient settings per 2025 updates), and company policies. Conducts comprehensive evaluations, creates and refines personalized treatment plans, supervises physical therapist assistants (PTAs) and aides effectively, contributes to interdisciplinary case management and care coordination, achieves superior patient outcomes, and cultivates strong, proactive relationships with facilities, referring providers, and clients to drive service quality, access, and growth.

Essential Functions

• Fully adhere to the applicable state Practice Act, including scope of practice, supervision standards for support personnel, and delegation guidelines.
• Annually review and comply with the company's Clinical Operations Policies & Procedures Manual.
• Complete all required Medicare compliance training and other regulatory or company-directed education.
• Complete annual OSHA training and strictly follow OSHA standards and pertinent governmental regulations.
• Perform detailed patient evaluations to identify impairments, functional limitations, and skilled therapy needs; establish medical necessity.
• Design, implement, progress, and modify individualized, evidence-based treatment plans aimed at restoring mobility, managing pain, enhancing function, preventing secondary complications, and promoting long-term independence.
• Provide skilled therapeutic interventions and maintain thorough, ongoing documentation that substantiates functional goals, measurable progress, skilled care rationale, and medical necessity.
• Prepare and submit all required documentation (e.g., evaluations, daily notes, progress reports, pre-authorizations, physician correspondence, charge entries) accurately, legibly, and promptly to satisfy facility, referring provider, and payer timelines and standards.
• Promote consistent, respectful, and collaborative communication with interdisciplinary team members, physicians, patients, families, and third-party payers.
• Participate actively in facility meetings, committees, and quality initiatives (e.g., utilization review, infection prevention, emergency preparedness) as assigned by the Center Manager.
• Complete continuing education sufficient to fulfill state licensure renewal requirements for each reporting cycle.
• Maintain current CPR certification compliant with state licensure standards.

Additional Responsibilities

• Manage time and resources efficiently to achieve or surpass facility productivity goals without compromising care quality.
• Contribute to and lead clinical in-services, staff training, and knowledge-sharing activities as directed.
• Remain up-to-date on evolving healthcare policies, regulatory changes (e.g., Medicare updates), evidence-based advancements, and trends in physical therapy practice.
• Assume meaningful leadership in selected center operational initiatives as agreed upon with the Center Manager.
• Undertake other duties or special projects as assigned.

Physical Requirements

• Frequently lift, transfer, and reposition patients using safe biomechanical techniques and body mechanics throughout the workday/shift; must independently handle up to 50 pounds.
• Constantly perform walking, standing, bending, squatting, reaching, kneeling, and other physical demands during the shift.
• Visual acuity (near and far) adequate for patient assessment, condition monitoring, record maintenance, individual recognition, and interpreting directions/instructions.
• Sufficient hearing and speech to comprehend detailed instructions, communicate clearly, and interact effectively in diverse settings.
• Ability to push/pull wheeled equipment (e.g., treatment carts, wheelchairs) across the facility and community.
• Fine motor coordination for precise manipulation of therapeutic/diagnostic tools, accurate documentation (manual or electronic), scheduling, and clinical procedures.

Required Credentials:
• Graduation from a physical therapist education program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) or, in the case of an internationally educated physical therapist, from a non-CAPTE accredited program seeking licensure as a physical therapist, a substantially equivalent education.
• State licensure/registration where applicable
