Job Responsibilities:

• Field-based, making Primary Care House Calls.

• Conduct comprehensive assessments, develop care plans, and provide continuous care for older adults in their homes.

• Collaborate with a multidisciplinary team to deliver integrated care.

• Utilize your expertise in geriatrics and primary care to make a meaningful difference in the lives of your patients.

• Fluent Spanish is ideal.

Qualification/Experience:

• Valid Georgia ARNP license.

• Reliable transportation and current state driver's license and proof of auto insurance.

• Current DEA license.
