At Aviata at West Palm Beach, we are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team! As a Licensed Practical Nurse, you will play a vital role in providing exceptional, person-centered care to our residents and patients.

As a Licensed Practical Nurse (LPN) at Aviata at West Palm Beach, you will:

• Provide direct patient care to residents and patients, assisting with activities of daily living under the direction of a professional nurse.
• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services and conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment.

Responsibilities:

• Assist with administering medications and treatments as directed by the resident's treatment plan.
• Monitor and report changes in resident condition to the Nurse Supervisor and identify potential safety hazards and take steps to mitigate them.
• Participate in quality assurance activities and quality improvement initiatives.
• Maintain accurate and detailed records of resident care and treatment.
• Collaborate with other healthcare professionals to deliver comprehensive care.

Requirements:

• A current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Ability to work a flexible schedule, including evenings, weekends, and holidays.
