Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families
• Develop individualized care plans in collaboration with the interdisciplinary healthcare team
• Provide emotional support, crisis intervention, and counseling services
• Coordinate discharge planning and facilitate smooth transitions of care
• Connect patients and families with community resources, financial assistance, and support services
• Advocate for patients' rights and ensure access to appropriate healthcare services
• Collaborate with physicians, nurses, case managers, therapists, and other healthcare professionals
• Assist with advance directives, guardianship concerns, and end-of-life planning when appropriate
• Maintain accurate and timely documentation in the electronic medical record
• Educate patients and families on available healthcare and community resources
• Ensure compliance with hospital policies, state regulations, and professional social work standards

Requirements

• Bachelor's or Master's Degree in Social Work from an accredited program
• Active Oregon Social Work license, if required by state regulations
• Minimum 2 years of recent healthcare or hospital social work experience
• Experience with discharge planning, case management, and community resource coordination
• Strong knowledge of healthcare regulations, insurance programs, and community services
• Excellent communication, interpersonal, and counseling skills
• Strong critical thinking, organizational, and problem-solving abilities
• Ability to manage multiple priorities in a fast-paced healthcare environment
• Experience with electronic medical record (EMR) systems preferred
• Commitment to providing compassionate, patient-centered care
