Discharge & Forensic Care Coordination Lead
Southside Behavioral Health
Southside Behavioral Health (SBH) seeks a non-exempt discharge planning liaison to coordinate services for individuals admitted to state hospital facilities or involved with the court system. This role requires writing individualized service budgets, contracting with providers, and reporting on funded services.
