On Call Visiting Nurse

Hartford Healthcare seeks an On Call Visiting Nurse to provide high-quality, patient-centered care to home-based patients during evenings, nights, weekends, and holidays. In this field-based role, you will perform skilled nursing assessments, manage symptoms, administer treatments and medications, and coordinate care with physicians and the interdisciplinary team. You will educate patients and families, support safe transitions of care, and document accurately in the EMR.
