Job Title: Hospice RN Case Manager

Location: Grand Junction, CO, United States
Job Type: Contract
Shift: Day Shift | Monday–Friday | 8:00 AM – 5:00 PM
Duration: 12 Weeks

"Navitas Healthcare, LLC" is seeking an experienced and compassionate Registered Nurse Case Manager to provide high-quality hospice and end-of-life care. This role focuses on patient-centered care, symptom management, family education, and coordination of hospice services across multiple care settings.

• Manage and coordinate individualized hospice care plans
• Perform patient assessments and symptom management evaluations
• Educate patients and caregivers regarding treatment goals and disease progression
• Coordinate interdisciplinary hospice care services
• Provide nursing care in homes, assisted living facilities, nursing homes, and group homes
• Support patients and families throughout all stages of hospice care
• Maintain accurate documentation and patient records
• Collaborate with physicians, social workers, and hospice care teams
• Ensure compliance with hospice care standards and safety protocols

• Active Registered Nurse (RN) License required
• Minimum 2 years of nursing experience required
• Hospice or case management experience preferred
• Strong communication, assessment, and critical thinking skills
• Ability to work independently and within interdisciplinary teams
