**Join Our Team as a Dynamic Case Management RN!** **Job Description:** - **Position:** Case Manager RN - **Req ID:** 63723 - **Location:** Alaska (Must have AK RN license in hand for Auto Offer) **Key Highlights:** - **Shift:** Day 5x8-Hour (Flexible start times: 0700-1530, 0730-1600, 0800-1630, 0830-1700) - **Experience Required:** Minimum of 1 year in an acute care setting - **Certifications Needed:** - AK RN License (required for Auto Offer) - BLS (preferred) - ACM Certification (preferred) **Responsibilities:** - Manage discharge planning for patients across various age groups, including neonates, infants, adolescents, adults, and geriatrics. - Collaborate with interdisciplinary teams, including: - IV Teams - Physical Therapy - Respiratory Services - Social Services - Rapid Response Teams, and more! - Conduct needs assessments and order Durable Medical Equipment (DME) as necessary. - Develop and manage comprehensive plan of care. - Utilize Milliman Guidelines to ensure best practices and quality care. - Comply with regulatory standards including CMS and HIPAA guidelines. **Patient Ratios:** - Days: 1:50 **Additional Information:** - **Weekend Coverage:** Every other weekend required - **On-call Requirements:** No - **Float Policy:** Yes, within case management services to cover gaps as needed - **Orientation:** 24 hours **Facility Insights:** - **Daily Census:** 324 - **Number of Beds:** 375 - **Work Setting:** Acute Hospital Case Management/Utilization Review **Requirements Summary:** - **Must Have:** - Epic EMR experience - Acute care hospital experience - AK RN License - **Preferred:** - Experience with Microsoft Office/Teams - Pediatric and Women’s Services experience (a plus!) **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
