Community Medical Centers has been providing central California with the highest level of care available in the region. Community Regional Medical Center measures up with top hospitals nationally and is the third largest in California! With 685 beds, we are home to the only comprehensive Burn and Level I Trauma center between Los Angeles and Sacramento. The 56,000 square-foot emergency department is one of the largest and busiest in California! We have an 84-bed Level III NICU and plan to significantly expand pediatric specialty care. Our hospital serves 2.5 million people within nine counties, averaging a daily census of 641 patients. We're affiliated with UCSF as a teaching facility; trained 3,000 Medical Doctor's to date plus continuous invests in research and training grants.

Responsibilities

Provides for delivery of basic pharmacy services in the Ambulatory Care setting. Reviews all drug orders and checks all work completed by ancillary personnel in the department. Provides medication consultation and drug information at point of sale. Serves as drug expert and consultant to physicians, nursing staff and other hospital personnel. Performs drug utilization review on all prescriptions including monitoring for inappropriate controlled substances usage. Accountabilities include:

• Performs drug utilization review on all dispensed medications reviewing patient profile for allergies, interactions and age appropriateness.
• Intervenes with provider when appropriate for clarification and correction of medication orders. Pursues opportunities for insurance formulary adherence to enhance patient compliance.
• Reviews and verifies all drug order entry, packaging and labeling by Pharmacy Technicians to assure accuracy and appropriateness for all dispensed medications.
• Directs pharmacy support personnel in the prescription dispensing process to ensure accuracy and timeliness.
• Prepares extemporaneous compounded medications as described in the non-sterile compounding policy and procedures.
• Maintains controlled substance records and documentation in accordance with local, state and regulatory law.
• Analyzes prescribing trends for narcotics and controlled drugs with CURES website to monitor patient compliance and to prevent excessive usage or harmful interactions.
• Provides medication consultation and drug information at point of sale. Addresses conflicts with customers as necessary.
• Serves as a drug expert and consultant to physicians, nursing staff and other personnel.
• Orders controlled substance medications from wholesaler and non-controlled medications when buyer is not available.
• Be involved in a specialty area of the pharmacy as assigned and provide that expertise to other staff.
• Takes part in the quality assurance program to improve workflow and reduce medication errors.
• Completes assigned individual projects to improve overall performance of the pharmacy.
• Assists in the training and mentoring of pharmacist and pharmacy technician students.
• Performs other duties as assigned.

Qualifications

Education: Bachelor's Degree in Pharmacy required. Doctor of Pharmacy (PharmD) preferred. Experience: Two years of Ambulatory Care Pharmacy experience preferred. Licenses and Certifications: RPH - Registered Pharmacist with valid State of California Pharmacist License required.
