Hospice Registered Nurse

Position Highlights Department: Home Care Hospice Hours: 30.00 per week Shift: Shift 1, Days Position Summary Applies the nursing process with patients and families, integrating principles that reflect sensitivity to their age-specific/developmental needs, gender, culture, religion, psychosocial and socioeconomic backgrounds in order to provide evidence-based, holistic care in a manner that respects the worth and dignity of each individual. Practices professional nursing as a registered nurse within the legal and ethical framework established by the Connecticut Nurse Practice Act and the American Nurses Association Scope and Standards for Nursing Practice and Code of Nurses, American Nurses Association Home Health Nursing, National Hospice Association and applicable regulatory standards. Position Responsibilities Obtains objective and subjective data about the health status of patients through history taking, physical examination, communication with health care providers and significant others. Identifies accurate nursing diagnosis/patient problems or needs from assessment data. Individualizes plan of care to address unique patient needs. Prioritizes plan of care based on needs. Collaborates with the patient, family and members of the health team to determine desired outcomes of care. Uses appropriate nursing judgment to prioritize and organize care. Implement evidence based patient care in a safe and effective manner. Collaborates with other members of the interdisciplinary team to develop a cost effective, comprehensive plan of care and goals according to changing needs of patient and family, changing settings and location of level of care. Minimum Qualifications A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice in the State of Connecticut. Has the knowledge and skills necessary to assess health needs of patients and to perform nursing care in a safe, caring manner.
