Impactful Child & Youth High-Needs Care Manager
Coordinated Care Services, Inc.
The CHHUNY High Needs Care Manager acts as the central point of contact for coordinating children’s and youth services. The role ensures providers collaborate with the child/youth’s best interests in mind and conducts comprehensive assessments to inform care planning.
