Highlights Pay: $66,575 - $142,576/year Location: Bureau County, IL Residency: Must reside in the state of Illinois Travel: Willing and able to travel up to 75% of the time What You'll Do Develop a proactive plan of care to enhance member outcomes. Conduct evaluations of member needs using clinical tools and data review. Apply clinical judgment to reduce risk factors and address complex health indicators. Conduct assessments considering various information sources, including claims. Use a holistic approach to determine the need for referrals to clinical resources. Collaborate with supervisors and stakeholders to overcome barriers in care. Present cases at interdisciplinary case conferences. Utilize motivational interviewing skills to engage members effectively. Interact with members/clients telephonically or in person. Meet with members/clients in their homes or worksites for ongoing case management.
