PROGRAM OPERATIONS MANAGER, POST ACUTE CARE

Program Operations Manager, Post-Acute Care 100% FTE. Primarily Remote – Onsite as business needs require. Position Highlights Provides operational support for the UW Medicine Post‑Acute Care (PAC) Program and assists PAC leadership in executing business and operational strategies for growth, development and evaluation. Primary Job Responsibilities Build operational infrastructure for efficient information sharing and deliverables tracking with internal and external stakeholders. Work closely with nursing leadership at UW Medicine hospitals and PAC Network partners to evaluate nursing practice and extend standard processes across the continuum. Partner with subject matter experts to develop project charters, roadmaps and detailed work plans that enable the PAC Program to achieve defined goals. Manage and execute multiple projects according to project plans and timelines; provide management, coordination, direction and support to the PAC team and implement tasks as identified by the project plan. Lead the design, creation, validation, analysis and communication of data and dashboards related to PAC priorities, in partnership with finance, analytics and other relevant teams. Maintain professional relationships with stakeholders at all UW Medicine entities and PAC Network partners including Skilled Nursing Facilities, Home Health Agencies, Hospice Services, Adult Day Health programs and Home Care Agencies. Manage marketing activities for PAC (e.g., updates to public‑facing website, flyers, development of SharePoint intranet site). Requirements Bachelor’s degree. Two to three years of experience working in a large health system with multiple EHRs, medical staff models and an array of clinical services. Demonstrated experience on quality and process improvement projects with cross‑functional teams, including data collection, analysis and reporting. Superior organizational skills with demonstrated ability to multi‑task and manage multiple projects while maintaining clear attention to detail. Ability to work independently with minimal supervision and manage multiple priorities. Strong collaborative, proactive approach to job functions; anticipates problems and seeks solutions. Proven record of well‑developed analytical, communication, interpersonal and problem‑solving skills, including tolerance for ambiguity. Significant and demonstrated proficiency in Microsoft Office applications and ability to learn new computer/software skills quickly. Customer orientation – establishes and maintains long‑term relationships, building trust and respect by consistently meeting and exceeding expectations with internal and external customers, vendors and stakeholders. Ability to understand and interpret regulatory standards, clinical guidelines, national quality metrics and health services research, including post‑acute care. Displays professional, ethical conduct and maintains confidentiality of information. Equivalent experience may substitute for degree requirement. Compensation and Benefits Pay Range Minimum: $100,368.00 annual. Pay Range Maximum: $120,444.00 annual.
