Daisy Private Home Care is seeking experienced Registered Nurses to complete client assessments, reassessments, care-plan updates, and supervisory visits throughout Jefferson, Shelby, and St. Clair Counties.

We are also accepting RNs interested in serving Tuscaloosa, Montgomery, and surrounding Alabama communities.

The RN will support individuals receiving home- and community-based services, including veterans and participants served through the Alabama Department of Mental Health.

This is a flexible, per-visit independent-contractor opportunity. RNs may select the geographic areas and assignments they are available to accept.

CONTRACT STATUS

Independent contractor — 1099

COMPENSATION

• $45 per completed supervisory visit
• $65 per completed reassessment or substantial care-plan update
• $85 per completed initial assessment and care plan

The RN will receive the assignment location, visit requirements, documentation expectations, and compensation information before accepting each assignment.

• Jefferson County
• Shelby County
• St. Clair County
• Tuscaloosa County and surrounding communities
• Montgomery and surrounding communities
• Additional nearby Alabama counties

RNs may select one or more preferred service areas and are not required to accept every offered assignment.

CORE RESPONSIBILITIES

• Complete initial nursing assessments and client reassessments
• Conduct required RN supervisory visits
• Assess client needs, risks, functional abilities, and changes in condition
• Develop, review, and update individualized care plans
• Evaluate whether services are being provided according to the approved care plan
• Observe caregiver performance during supervisory visits
• Identify caregiver training or corrective-action needs
• Provide appropriate clinical guidance within the RN’s scope of practice
• Communicate concerns and recommendations to agency leadership
• Complete clear, accurate, and timely clinical documentation
• Report significant changes in condition, incidents, and safety concerns
• Maintain client confidentiality and follow applicable agency and payer requirements
• Participate in care coordination when required

QUALIFICATIONS

• Current, unrestricted Alabama Registered Nurse license
• Minimum of two years of nursing experience preferred
• Home care, home health, geriatrics, veterans’ care, behavioral health, or intellectual/developmental -disability experience strongly preferred
• Experience completing assessments, care plans, or supervisory visits preferred
• Current CPR or BLS certification
• Professional liability insurance
• Valid driver’s license, automobile insurance, and dependable transportation
• Ability to travel within the RN’s selected service area
• Ability to pass required background and exclusion checks
• Strong assessment, documentation, communication, and time-management skills
• Access to a smartphone, tablet, or computer for electronic documentation

SCHEDULING

Visits are scheduled according to client and agency needs. RNs may provide their availability and select assignments based on location, schedule, and visit type.
